This agreement is entered into between the participant (and parent/guardian, if applicable) and Rapadura Jiu-Jitsu (the “Academy”).
Assumption of Risk
I understand that participation in martial arts activities involves physical exertion and contact, and may include inherent risks such as falls, impacts, strains, and accidental injury. I voluntarily assume all risks associated with participation in the birthday party event and related activities.
